Collection of Form K (BORANG K)

After 191 days, 6.3 months (counting from the day of my MY-RN1 Acknowledgment Slip collection). I received an email from Consular Division, High Commission of Malaysia, stated that I can collect the BORANG K and birth cert from the Consular within 1 month from the date of that email.

This is in fact the last step from the Malaysia side. For Singapore ICA, I would need to upload the BORANG K to their website once I got the physical copy from the Malaysia Consular.

Dear Sir/Madam,

RE: RENUNCIATION OF MALAYSIAN CITIZENSHIP – COLLECTION OF FORM K

Kindly be informed that your application for renunciation of Malaysian Citizenship has been approved. Please come personally to collect your FORM ‘K’ / Birth Certificate from the Consular Section between 2.30pm to 4.00pm only, Monday to Friday within one (1) month from the date of this email.

 

2.         Kindly print this email and bring along the following documents (Original & two (2) clear photocopies each in A4 size paper - to be photocopied beforehand):

i.        Singapore Identity Card or 11B Card (to photocopy front and back of document); the temporary slip will not be accepted;

          ii.       Singapore Passport (bio-data page only);

 

iii.       Singapore Citizenship Certificate (to photocopy front and back of

document);

          iv.      Deed Poll/Baptism (if name has been changed), if applicable

          v.       Acknowledgement Slip (MY-RN1 Form)

**Photocopy and photo booth services are not available at our premise at the moment.  Kindly ensure all documents required are ready before your arrival.

For those who are still holding temporary Singapore NRIC, please contact ICA Singapore and request for a certified copy of your Singapore NRIC and citizenship certificate.

You may use the certified copy of the said documents to collect the approved Form K / birth certificate from the High Commission.

3.         Collection on behalf of the applicant (strictly for family members or spouses):-

      i.          Original Authorisation Letter from the applicant

      ii.         2 copies of identity card of the authorised person

      iii.        All the photocopy documents from Paragraph 2 (i, ii, iii, iv, v)
